His heart is racing. He never thought that John might be a victim as well - at least not now that the game was almost over.
So lost he was in the thrill of the game that it clouded his judgement, his rationality; that he forgot of the almost part.
He should have been able to see this coming, to even think of it as a possibility.
Now John is in front of him, wearing a jacket full of bombs. Sherlock is disgusted with himself, with his lack of reasoning.
His heart cannot stop racing and it might burst out.
